The Life Group Pastoral Care Report helps to keep the pastoral team informed about our Life Groups and the needs that people have. Please fill out and submit the form below after each Life Group meeting. You can also print the Life Group Report Form and drop the completed form in Pastor Joe's mailbox in the church office or fax it to 508-653-8494.

Examples of situations that pastors need to be aware of: Death or terminal illness, abusive situations, severe marriage difficulties, change in emotional health, new babies, hospitalization, loss of employment.

Pastoral Care reporting is not intended to interfere with people, but to assist them in their crisis. The Senior Pastor wants to be informed about the state of the flock and be ready to help anyone in need. Your communication is vital to the pastoral team.
